By signing this declaration, I confirm that:

I have read the Terms & Conditions of UPREFORMER and accept that workouts take place without an instructor or medical professional physically present.
My child is in good health, shows no signs of illness and has no medical contraindications to using a Pilates reformer.
I fully understand the nature of the physical effort involved and the potential risks of using a Pilates reformer independently.
I take full responsibility for the safety, actions and behaviour of my child while they are in the studio.
I declare that I will bring no claims, actions or demands for compensation against IVELAS Group EOOD, its owners or its staff in the event of a contusion, injury or incident.
(For children aged 12 to 14.) I undertake to personally accompany my child in the studio throughout the entire training session.

I give explicit consent for the studio to process the health data about my child stated in this declaration.
